A research team in digital pathology ran a laboratory study published in Revista Española de Patología¹ comparing 10 different pointing devices, including RollerMouse, vertical and conventional mice.

While people did the same on-screen task, they measured upper-arm and forearm muscle effort using surface electromyography.

They found two big things:

RollerMouse required the least effort. Less muscle effort means less day-to-day strain, the sympthoms that often feels like aching or fatigue.
